CalAIM is a multi-year initiative to transform California’s Medi-Cal program with broad delivery system, program and payment reforms. Two core programs of CalAIM are Enhanced Care Management (ECM) to provide case management for certain members with complex needs and Community Supports to deliver services that meet health and health-related needs.
Enhanced Care Management (ECM) is a case management benefit that certain members with complex needs can qualify for by meeting Population of Focus criteria. Below includes ECM policy explainers and member outreach tools to support member enrollment.

This section contains relevant guidance for contracted ECM and Community Supports providers from the Department of Health Care Services (DHCS). There are standardized DHCS program requirements from DHCS that all Medi-Cal managed care plans must comply with, but plans also have the flexibility to set their own program policies in certain areas.

Effective patient-centered care requires patient data to be safely shared between health and social service providers to coordinate care and develop an understand of a patient’s full array of needs. Alameda County uses a Community Health Record (CHR) that pulls data from across local organizations through the Social Health Information Exchange (SHIE) for this purpose. If you are not already doing so, consider connecting with the CHR and SHIE to access the data you need to best serve your patients and clients.
Sharing of health and social service information is a core component of CalAIM to coordinate services across providers. To advance interoperability across the state, California’s Data Exchange Framework (DxF) will require most health care organizations to begin sharing data with each other in accordance with the Data Sharing Agreement (DSA) and Policies & Procedures (P&Ps) by January 2024. This section includes key information on DxF, plus additional resources to help local providers share data to support CalAIM.
With new Medi-Cal Managed Care contracts going live in 2024, Anthem will be transitioning out of the county as Kaiser Permanente enters to join the Alameda Alliance for Health (AAH) in serving Alameda County. Your PATH facilitators are monitoring the transition period to ensure a seamless transition for Medi-Cal members receiving ECM and Community Supports services.
