BluePath Health is a trusted vendor in California’s TA Marketplace, providing no-cost strategic support to Medi-Cal providers to enhance whole person care, and to improve access, equity, and health outcomes statewide.
Since its founding in 1984, 2-1-1 Orange County (211OC) has been instrumental in linking residents with critical social services, including housing, job placement, food, and health insurance.
BluePath Health and Orange County United Way are currently focused on aligning data sharing capabilities and coordination to best support CalAIM services, such as Enhanced Care Management (ECM) and Community Supports (CS).
The Alameda County Community Food Bank (ACCFB) envisions a hunger-free community where no resident has to worry about their next meal. To help advance this mission, BluePath Health supported ACCFB with a Phase I CalAIM readiness assessment and workflow design, improving service delivery efficiency and resource management.
Omatochi is revolutionizing in-home senior care by prioritizing independence and holistic wellness. BluePath Health has supported Omatochi’s CalAIM outreach efforts by facilitating community events, and strengthening referral networks. This includes conducting a comprehensive Community Services Capacity Assessment through 20+ local provider interviews; managing and facilitating 14 Senior Wellness Fairs and a Care Workforce Summit; and promoting career pathways for caregivers while addressing healthcare access disparities and social isolation.
The Fresno Probation Department is committed to community protection while offering intervention programs for justice-involved youth. To align with CalAIM’s Justice-Involved Initiative, BluePath Health is conducting a landscape analysis of workflows and systems, identifying gaps relative to Enhanced Care Management (ECM) requirements, and coordinating with external stakeholders to support Fresno’s work to become an ECM provider.
MedZed partners with health plans to reach high-risk members who have complex healthcare needs. Since 2023, BluePath Health has been collaborating with MedZed to select electronic health record (EHR) and population health management systems and to improve Enhanced Care Management and Community Supports processes.
The California Food is Medicine Coalition (CalFIMC) provides medically tailored meals to low-income individuals with chronic illnesses. BluePath Health is spearheading a major initiative for CalFIMC to bolster its analytic capabilities. Components have included identifying an analytics vendor, identifying and contracting with QHIOs and other health information networks that are critical partners for CalFIMC organizations, and building critical data governance and HIPAA compliance capabilities to enable this work.
