The Tri-Counties Collaborative brings together CalAIM stakeholders from San Luis Obispo, Santa Barbara, and Ventura Counties to coordinate the implementation of Enhanced Care Management (ECM) and Community Supports. Meeting monthly, the group includes health plans, county leaders, providers, and community organizations. CenCal Health (serving San Luis Obispo and Santa Barbara) and Gold Coast Health Plan (serving Ventura) are the region’s Medi-Cal managed care plans. The collaborative is facilitated by BluePath Health.
