BluePath Health offers Technical Assistance (TA) through the Department of Health Care Services CalAIM TA Marketplace in the domains of Building Data Capacity and Supporting Cross-Sector Partnerships.
We are adept at organizing stakeholders in a community or region and assessing technology and process needs, aligning goals and objectives, then bringing forward actionable recommendations, often being accountable for assisting in the delivery of those recommendations. Our technical assistance services include education and knowledge building, in addition to implementation of change associated with a wide range of technology, quality improvement efforts and providing guidance to all levels of resources involved in a program.
Our team brings decades of experience working in community and statewide data exchange strategy development, as well as implementing and optimizing EHR systems, closed-loop referral platforms, and analytics and population health management platforms. We take a holistic approach in understanding program and technical requirements and integration constraints, and incorporate provider, community partner and patient experience. We have experience working in diverse provider and community-based settings, including support for rural FQHCs and public hospitals to incorporate new technologies (e.g. telehealth, electronic consults and referral management) into their processes.

Key Activities
- Project plan development and maintenance
- Facilitation of stakeholders and agencies involved in CalAIM activities, including meeting planning, agendas, minutes, action items
- Management of project risks and issues
- Coordination of project communications
- Mentoring team members on project management tool development and project management skills
Support for selection and procurement of a technology platform that may be introduced to a community.

Support to ECM and/or Community Supports providers with needs associated with submitting claims to MCPs for services rendered under CalAIM
- Provide revenue cycle/claims management overview and assistance
- Review claims system(s) and processes
- Document processes and map against requirements
- Provide guidance based on review of last 30-60 days of claims submission
- Recommendations associated with claims submission optimization e.g. data enhancement, management processes, access to information, accuracy of data, etc.
- Project planning and milestones development
- Key Performance Indicators (KPI) / Reporting Guidelines associated with utilization
- Workflow analysis and documentation within the service provider’s domain, including handoffs to other parties
- Onboarding support and training and materials that will be transitioned to long term project team
- Go Live support
- Provide required state workflow templates based on State guidelines
- Perform gap analysis of current state and required state mapping for pre-release JII activities.
- Provide data and system enhancement recommendations.
- Support development of MOUs where required.
- Engage agencies involved in JII re-entry transitions and advise on workflow change.
- Provide documentation containing workflow steps and hand-offs between health care providers, community supports and operations staff in support of JII eligibility, enrollment and re-entry with services support.

- Community readiness and gap assessment including evaluation of partners needs and technology map that will support further planning and implementation efforts. This covers aspects like process flows, services gaps, essential data sharing, staff training, and community engagement.
