Tri-Counties CalAIM PATH Collaborative Resource Center
The Tri-Counties Collaborative Planning and Implementation Group comprises San Luis Obispo, Santa Barbara, and Ventura Counties. The collaborative meets monthly to discuss CalAIM implementation and support the rollout of Enhanced Care Management (ECM) and Community Supports. Health plans, county leaders, community-based organizations, hospitals, clinics, and other CalAIM stakeholders are encouraged to join. The Medi-Cal managed care plans in this region include CenCal Health (San Luis Obispo, Santa Barbara) and Gold Coast Health Plan (Ventura). The Tri-Counties Collaborative is facilitated by BluePath Health.

Upcoming Collaborative Meetings
The Tri-Counties PATH Collaborative brings together health plans, county leaders, community-based organizations, hospitals, clinics, and other CalAIM stakeholders to discuss CalAIM implementation and the rollout of Enhanced Care Management (ECM) and Community Supports. The Collaborative is a space to share innovative strategies, problem-solve together, and build relationships to better serve Medi-Cal members in San Luis Obispo, Santa Barbara and Ventura counties.

CalAIM is a multi-year initiative to transform California’s Medi-Cal program with broad delivery system, program and payment reforms. Two core programs of CalAIM are Enhanced Care Management (ECM) to provide case management for certain members with complex needs and Community Supports to deliver services that meet health and health-related needs.

This section contains overviews on the latest ECM and Community Supports program updates from the Department of Health Care Services (DHCS).
Sharing of health and social service information is a core component of CalAIM to coordinate services across providers. To advance interoperability across the state, California’s Data Exchange Framework (DxF) will require most health care organizations to begin sharing data with each other in accordance with the Data Sharing Agreement (DSA) and Policies & Procedures (P&Ps) by January 2024. This section includes key information on DxF, plus additional resources to help local providers share data to support CalAIM.
